The Community Health Centre model works

Nova Scotia communities are caring and innovative, and we are experts in local priorities. But many Nova Scotians struggle to access the right health provider at the right time because of geography, complexity, stigma, and systemic barriers. The Community Health Centre model is unique because it is community-governed and focused on the upstream conditions for health in the community, such as food security, housing, and social inclusion.
